HTML and forms are not rocket science, neither are they as simple as pouring a glass of water. This primer will explain a little about how it all works. It may help you a bit in deciding exactly how to design your survey.
There are six types of user input available, Radio Button, Checkbox, Select List, Multiple Select List, Text, and Textarea. I usually pick an input type that seems appropriate to each question, but if you have a preference, just specify it in your subm ission.
In each example, there is no selected default answer. For all input types, preselection is possible but discouraged to avoid giving the appearance of 'leading' the user or 'forcing' them to answer a question.
Radio Button offers the reader two or more optional selections. Only one selection will be submitted as an answer. This That The Other
Checkbox is similar in that if offers the user multiple choices, but more than one item may be selected for their answer. This That The Other
Select List offers the user a more interactive method of selecting an answer from a menu. Like Radio Button, only one answer may be selected. To disable preselection, the default answer is 'blank'. This That The Other
Multiple Select List is similar to Select List, but like Checkbox the user may select more than one item for their answer by holding the <CTRL> key and pressing the mouse button. The full list is displayed so the user can see all the answers he's selected. This That The Other
Text is for simple, one line answers typed in by the user.
Textarea offers the user more space to enter their answer.
