I'm always looking for new ideas to add to thesurvey.com. If you have a survey that you would like to have added please describe it here. You must include your name and email address so I can get back to you.

This service is free for almost any educational or non-profit purpose.

Details about commercial surveys.

Here's how the process works:

1. Enter your data in the form below.
2. It takes a couple of hours to a few days to write a survey page. At that time it will not be available to the general public.
3. I will email you an URL for your survey. Please review and test it. Submit a few forms to see what the responses look like.
4. Email me to advise of any corrections or changes you feel necessary, or just say 'OK, good, go with it'.
5. When you are satisfied with the survey page I will:
   • Add your survey to the main menu.
   • Send an announcement out to a distribution list of people who want to be notified of new surveys, about 600 people as of April 2000.
   • Post an announcement to alt.usenet.surveys
6. I will not advertise your survey in newsgroups or mailing lists. My name/face is unfamiliar in the places you are used to discussing your topic. You should make it known in the places you are familiar with that the survey is available. Please do not spam inappropriate groups or lists, that will only make us both look bad, and I trust you to not do that.

Important: I will not install your survey until I have your approval that everything looks and works 'OK'.

Important: I will not advertise your survey. After your survey is up and running it's up to you do make it known that it's available. If you don't do that, very few people will know about it.

By proposing a survey, you are an integral part of the survey page. If you are not familiar with HTML, here are some technical pointers that may help you in deciding how to design your survey.

Important: Please double and triple check that your email address is absolutely correct. If there are any errors, my email to you will bounce. You will wonder why I never contacted you, there will be disappointment all around. Please double, triple, and quadruple check, this is actually the most important piece of data in this form.

Just a couple of sentences that say who you are, what you are doing, and the reasons/purposes for your survey.

Don't worry much about formatting, just list the questions in the proper order.

It's polite to say 'thank you' to survey participants. How would you like to say 'thank you' to the user?

Required Fields (Optional)
By default, all survey questions are 'optional', they are not 'forced' on the user. If there are any questions that are particularly important, that you would like to be 'required', please list them here. This option is strongly not recommended unless you feel it is absolutely necessary; users don't like participation shoved down their throats.

'Personalised' Surveys (Optional)
Do you have an existing WWW page that you would like to add a survey to? To more personalise your survey, the page at thesurvey.com can display an image from your page in the survey. After the user submits his survey response, the 'thankyou' screen can carry an optional link back to your page. Please list the URLs for an image and/or page you would like your survey to link to.
